Self-Employed Health Insurance in Illinois (2026)
Freelancers, contractors, gig workers and small business owners in Illinois buy their own coverage. Here’s how to get solid coverage at a price that works, and the tax rules worth knowing.

Self-employed in Illinois
If you work for yourself in Illinois, you buy coverage through Get Covered Illinois or directly from an insurer. Premium tax credits are only available through the Marketplace, and they are based on your estimated yearly income, so a careful income estimate matters.
Illinois has no state coverage mandate or penalty, and no extra state premium subsidy for 2026. You can also check a box on your Illinois tax return to share information with Medicaid and Get Covered Illinois, which can help you learn whether you qualify for help.
Because income can swing from year to year, update your Marketplace application if your earnings change a lot. That helps avoid a large repayment at tax time, especially now that the 400% cliff is back.
The self-employed health insurance deduction
If you have a net profit from self-employment, you may be able to deduct the premiums you pay for medical coverage for yourself, your spouse and your dependents. The deduction isn’t available for any month you were eligible for an employer-subsidized plan, including through a spouse’s job, and it can’t be more than your net earnings from the business.
If you also receive a premium tax credit, the two interact, and the calculation gets technical. Ask your tax preparer to handle it; we can help you pick the plan.
Health savings accounts
An HSA-qualified high-deductible plan lets you put pre-tax money into a health savings account that you own and keep, even if you change plans later. For healthy self-employed people with some cash reserves, pairing a lower-premium HSA plan with regular HSA contributions is often the most tax-efficient setup.
Illinois questions

No. Illinois does not have its own individual mandate or tax penalty. Coverage is still worth having, since a single hospital stay can cost far more than a year of premiums.
Not for 2026. Premium help in Illinois comes from federal premium tax credits, which you can only get through Get Covered Illinois.
Report the change to Get Covered Illinois so your tax credit can be adjusted. If you take too much help, you may owe some back when you file taxes.
