Bridging to Medicare in Illinois

Retiring before 65 in Illinois means finding coverage for the years before Medicare. For most people that is a Marketplace plan, and the price you pay depends heavily on the income you report for the year.

With the extra federal credits gone, 2026 brought back the cliff at 400% of the poverty level. If your income lands just above it, you can lose all premium help, so the timing of retirement account withdrawals, pension starts and Social Security matters. On average, Illinois customers who got financial help in 2026 paid about $140 a month.

If you are married and your spouse is still working, Illinois spousal continuation may help. A spouse who is 55 or older when the employee retires can keep the group coverage until reaching Medicare eligibility. Ask the employer for the details in writing, since the notice deadlines are short.

Why your income estimate matters so much

Marketplace subsidies are based on your modified adjusted gross income (MAGI) for the coverage year. For early retirees, that number is partly in your control: it depends on which accounts you draw from and when.

Withdrawals from traditional IRAs and 401(k)s count as income. Qualified Roth withdrawals generally don’t. For ACA purposes, MAGI also adds back tax-exempt interest and any Social Security benefits that aren’t taxed. A tax professional and a licensed agent working from the same income plan can make a large difference to what you pay.

Subsidies in 2026

The extra federal premium tax credits expired at the end of 2025 and were not renewed, so the 2026 plan year runs on the original ACA rules. That means the subsidy cliff is back: households above 400% of the poverty level generally get no premium tax credit, and Illinois rates rose an average of 28.8% for 2026.

Most early retirees use a Marketplace plan, possibly with premium tax credits based on household income. If your spouse is still covered at work, Illinois continuation or spousal continuation may also apply.

Yes. Tax credits are based on your yearly household income, and in 2026 the cliff at 400% of the poverty level returned. Planning withdrawals and when to start Social Security can keep more of your help.

If she is 55 or older when you retire and your employer's plan is an insured group plan, Illinois law lets her continue coverage until she is eligible for Medicare. After the first two years, an administrative fee may be added.
