Bridging to Medicare in Louisiana

Retiring before 65 means bridging the years until Medicare. For most Louisiana early retirees that bridge is a Marketplace plan, COBRA or state continuation for a while, or a spouse's employer plan.

Subsidies are based on the income you expect for the year, so how you draw from savings, retirement accounts and Social Security matters. Since the extra credits ended after 2025, going even a little over 400% of the poverty level can mean losing all premium help, so plan withdrawals carefully.

If your countable income is very low, Louisiana Medicaid covers adults up to 138% of the poverty level. Starting in 2027, most expansion adults will need to meet a work, volunteering or school requirement, so check how that applies to you.

Why your income estimate matters so much

Marketplace subsidies are based on your modified adjusted gross income (MAGI) for the coverage year. For early retirees, that number is partly in your control: it depends on which accounts you draw from and when.

Withdrawals from traditional IRAs and 401(k)s count as income. Qualified Roth withdrawals generally don’t. For ACA purposes, MAGI also adds back tax-exempt interest and any Social Security benefits that aren’t taxed. A tax professional and a licensed agent working from the same income plan can make a large difference to what you pay.

Subsidies in 2026

The extra federal premium tax credits ended on December 31, 2025, and Congress has not renewed them. In Louisiana that means smaller subsidies for many people and no subsidy at all above 400% of the federal poverty level. Paid enrollment in February 2026 was about 26% lower than a year earlier, largely because of this change.

It depends on your expected income for the year. Since the extra credits ended after 2025, household income above 400% of the federal poverty level gets no tax credit at all. Below that line, help can still be large, so planning withdrawals matters.

Yes, if household income is at or below 138% of the federal poverty level. Starting in 2027, most expansion adults will need to show work, volunteering, school or job training of about 80 hours a month, so check how that affects you.

You should sign up for Medicare when you first become eligible and then end your Marketplace plan. Tax credits generally stop once you can get premium-free Medicare Part A.
