Self-Employed Health Insurance in Iowa (2026)

Freelancers, contractors, gig workers and small business owners in Iowa buy their own coverage. Here’s how to get solid coverage at a price that works, and the tax rules worth knowing.

Self-employed in Iowa

Self-employed Iowans, including many farm families, buy coverage through the Marketplace like any other individual. Your subsidy is based on your household's projected income for the year, which for a business owner is generally net self-employment income, not gross sales.

Iowa has no state coverage mandate or penalty, and no extra state premium subsidy, so your costs come down to the federal tax credit and the plan you choose. With six insurers in 2026, compare networks carefully: Wellmark has most of the market, and smaller insurers may have narrower networks in your county.

If you are a Farm Bureau member, the Farm Bureau Health Plan is another option, but it uses medical underwriting and does not qualify for premium tax credits. Compare its price to your after-subsidy Marketplace price before you decide.

The self-employed health insurance deduction

If you have a net profit from self-employment, you may be able to deduct the premiums you pay for medical coverage for yourself, your spouse and your dependents. The deduction isn’t available for any month you were eligible for an employer-subsidized plan, including through a spouse’s job, and it can’t be more than your net earnings from the business.

If you also receive a premium tax credit, the two interact, and the calculation gets technical. Ask your tax preparer to handle it; we can help you pick the plan.

Health savings accounts

An HSA-qualified high-deductible plan lets you put pre-tax money into a health savings account that you own and keep, even if you change plans later. For healthy self-employed people with some cash reserves, pairing a lower-premium HSA plan with regular HSA contributions is often the most tax-efficient setup.

No. Iowa has no state individual mandate, and the federal penalty is $0. You can still face large bills if you get sick without coverage, so it pays to compare options during open enrollment.

It is worth comparing if you are an Iowa Farm Bureau member and in good health. The plan is underwritten and does not take premium tax credits, so if you qualify for a subsidy a Marketplace plan may cost less. People with health conditions can be declined.

Wellmark Health Plan of Iowa, with about 80,000 individual members as of spring 2026. Oscar is second with about 12,000. Check that your doctors are in network before choosing any plan.
